The purpose of this website is to inform patients, families and/or caregivers about the role of this medical home and provides patients, families and/or caregivers with materials that contain all relevant information on our practice.

A medical home is a patient-centered approach to primary care that makes healthcare accessible, family-focused, and consistent from infancy to young adulthood. It ensures comprehensive care, coordinating between providers and respecting your family's culture and values. More than just a place, it’s a partnership between your family, healthcare professionals, and the community to support your health.

Our office is conveniently located at 2116 Bath Ave, Brooklyn, NY 11214.

Our operating hours:

Monday: 2pm - 8pm

Tuesday: 2pm - 8pm

Wednesday 2pm - 8pm

Thursday: 2pm - 8pm

Friday: 2pm - 8pm

Saturday: 2pm - 7pm

We accept same day appointments each day of the week between 2pm - 8pm. We encourage patients to call to get a same day visit.

If you need to reach us after hours please call the office phone number and the answering service will direct your calls to the relevant providers.

  • Dr. Mohammad Kamran Ahmed

    Dr. Mohammad Ahmed, MD is a family medicine physician in Brooklyn, NY and has over 30 years of experience in the medical field. He graduated from Dow Medical College Karachi in 1995. He is affiliated with medical facilities such as NYU Langone Hospitals and NYU Langone Hospital-Brooklyn. He is accepting new patients and telehealth appointments.

  • Dr. Arshad Mahmood

    Dr. Mahmood is Board Certified in Family Medicine. He graduated from Long Island University in 2025. HE is dedicated to providing compassionate, comprehensive care for every patient and a huge proponent of team based care. He practices primarily in Brooklyn. He is accepting new patients and telehealth appointments.

Patients can call in to select the doctor of their choice for their appointments.
